Part I Application

Submit this on-line form to begin the YASC application.  Download Part II application and reference forms here.  
Application is complete when part 1, part II and 3 completed reference forms are received by UCC Volunteer Ministries office, blaufusm@ucc.org or 700 Prospect Avenue, Cleveland OH 44115

Placement process begins as completed applications are received.  
Early decision deadline December 15, 2013.  
All completed applications due before February 15, 2014.


Additional placements will be made on a rolling basis until all positions are filled.

At the end of this on-line form, download part II of the application and reference forms.  Distribute forms for three people to complete and return to the UCC Volunteer Ministries Office.  A reminder with part II and reference forms attached will also be e-mailed to you.   If you do not have access to e-mail, please contact the UCC Volunteer Ministries Office for a printed copy of this application at 216-736-3216.

Application will be considered complete when parts I, II and reference forms have been returned to:  UCC Volunteer Ministries Office, 700 Prospect Avenue E., Cleveland, OH 44115 or completed, scanned and e-mailed to blaufusm@ucc.org.
