Below is a form for volunteers who have recently ended their service.  This form gives the Executive of Volunteer Ministries valuable input on the services this Ministry provides.  End of Service Reflection is also available in Word.

Reflection on the work environment.  (Reflections on your orientation to the placement site when you began and training opportunities available throughout the time of service.  Was the work-related supervision sufficient?  What skills and talents have you developed as a volunteer and are there ways in which you would like to develop these further?)
Reflections on social-spiritual support. (Comments on housing, food, stipends ... Who did you contact when you needed someone to talk to and was this sufficient for you?  Did you find a community to relate to?  Reflections on the meaning of service for you now.)
Describe and Reflect on a special experience that might have transformed your life or the life of someone else.
Future Volunteers (Would you recommend that the Office for Volunteer Ministries place another volunteer at your placement site?  What counsel and preparation would be helpful for future volunteers?)
