F.A.S.T. Brochure

1. Submit this form to request your week for the F.A.S.T. program.

2. The cost is $225.00 per person. Children 11 and under are $125.00 per week. Fee includes all program costs, building materials, daily sack lunch, and F.A.S.T. T-shirt. Planning, purchase and preparation of breakfast and dinner will be the responsibility of each group. Kitchen facilities are available on-site.

3. A $40.00 non-refundable deposit per person must be received within a week of completing the on-line registration form. Until the deposit is received, the week is not held and can be given to another group!

4. You will receive a confirmation by e-mail securing your date when your deposit is received. Prior to taking your trip, individual registration forms and waivers will need to be completed and returned to us by April 17. The balance due for your trip is required by May 15.

5. After your group has registered, you can add participants if space is available. To add participants to your group, check for availability each week. Contact Brenda Tyler at brenda@dhm.disciples.org or by phone: (317) 713-2642 or (888) 346-2631. If space is available, participants will be added upon receipt of the additional deposit.

6. You will receive planning and preparation materials in mid-February.

Remember that you must pay a non-refundable deposit of $40 for each spot you reserve. We cannot return any of the deposits as we turn groups away based on how many spots you reserve.
