This brief survey takes the pulse of your congregation.  Like a pulse, it gives only a rough estimate of how strongly your "heart" is beating.  For a more detailed picture and for congregational vitality resources, check out the Ready, Set, Grow Resource Modules on this www.ucc.org/vitality website.

Print out and complete this version of the Vital-O-Meter to rate your congregation's "pulse."
 
Score each item with Strongly Agree = 4, Agree = 3, Unsure = 2, Disagree = 1, and Strongly Disagree = 0.
 
Then add up the value for all six questions for a total score.
 
The total score theoretically can range from 0 to 24, though probably most will score above 12.
 
Here's how to evaluate your score:
