Below you will find links to various Yearbook forms. Simply click on the link to each form. The form should open in your browser window. You can either complete the forms on your computer and print them and return to your Conference or Association. Or you can just print them and fill them out by hand. If you'd like to save the form to complete later, click on the button that looks like a computer disk. A dialogue box will appear to save the file to your computer.
Please be sure to download the Local Church Instructions for instructions on completing the Local Church Report (Form G), Church Officers (Form E) and Ministerial Support (Form F). You should have received the Form G from your Conference or Association. If your Conference has decided to make the online reporting system available for Form G you will receive separate instructions from them.
You will need Adobe Reader to open all these forms. If you do not have this, please go to http://get.adobe.com/reader to download it for free. Follow the instructions located at this website for downloading and installing Adobe Reader to your computer.
